Dr Cook has been with
CHOA since January
1999
after retiring from the
U.S. Army. He attended the Univ of Kansas
where he obtained his MD, PhD. Dr Cook's internship and first year of
residency training was at Madigan Army Medical Center with the completion
of his residency and fellowship in Hematology Oncology at Walter Reed
Army Medical Center. Following the completion of his training he was
assigned to Tripler Army Medical Center in Honolulu from 1982-1992 and
again from 1995 until his retirement in 1999.
